When you come to the office please bring your pill bottles or an updated med list. Also if transferring to our office you need to sign a release for either here or at your current provider.
Also you must call your insurance provider to let them know Seneca Primary Care is your new PCP prior to your first appointment.
If any patient does not inform us of your updated proper insurance information at the time of service, you will be responsible for payment.
